- Intended to be printed and administered to students
- Student results submitted for scoring
- Must include 5 items at the same standard and Access Point
- A number of Final forms have been pre-built for each standard and complexity level Access Point. These forms are identified with the “Public” source. Public forms may not be edited in the Item and Form Management application.
- Test Administrators may also build their own Final Forms.
- The Test Administrator's name is automatically designated as the source for a Final Form created by the Test Administrator.
- Must be set to Published in order to be associated with a student for scoring in the Registration application.

- May contain more or fewer than 5 items
- May assess different standards, complexity levels, and content areas
- Cannot be Published. These forms do not appear in the Registration application and are not associated with specific students in the DRC INSIGHT Portal

Baseline |
- Designed to be a baseline and final assessment with a minimum of six weeks of instruction between the baseline and final administration
- Can be formal or informal and is intended to assist the educator in determining the appropriate complexity level to focus instruction, and ultimately help determine the best performance task complexity level to submit for scoring.
- Not submitted for scoring
- Are not restricted to 5 items per standard and Access Point
- May contain more or fewer than 5 items
- May assess different standards, complexity levels, and content areas
- Cannot be Published. These forms do not appear in the Registration application and are not associated with specific students in the DRC INSIGHT Portal
